Telehealth—UPDATE

While the COVID-19 crisis is no longer as severe and life-threatening as it was at the outset, our clinic still requires patients to switch to telehealth sessions if they likely have a contagious illness such as COVID, the flu, or even a bad cold. The pandemic allowed us to reprioritize health and also to make adjustments to how we practice so that we can help prevent the spread of illnesses and keep our staff and patients as healthy as possible. We are currently operating on a hybrid schedule with some in-person availability, but we request that all patients, regardless of how they are being seen, switch to telehealth if they are feeling under the weather, but still well enough to attend a session. Thank you, in advance, for your understanding.

Parking

The main doors to the building are located on Lincoln Avenue, just South of Belmont. Typically, there are plenty of metered parking spots available on Lincoln. If you wish to search for free parking, occasionally you can find a spot on a nearby side street. That said, there has been quite a bit of construction occurring at our intersection for the past year. We hope this construction is completed soon, but it is a good idea to allow yourself extra time if you need to park.

 

Intake Paperwork

Prior to your first session, you will need to complete new patient paperwork. The clinic director will email you a link to our client portal at the time of scheduling. Completed paperwork is required in order to attend your first session.

 

Privacy and HIIPA

 At our practice, your health information is considered confidential.  There are only a few scenarios in which your information may be disclosed without your permission.  These scenarios are discussed in the newest version of the Notice of Privacy Practices, effective September 23rd, 2013.  You may ask for a copy of these privacy practices at any time.  A link is also provided for you below. We encourage you to ask us any questions that you may have about your rights and our responsibilities regarding the security and privacy of your health information.

Please note that this brochure is the standard-issue notice of privacy practices.  Our practice differs in some ways:

  • We will never sell your health information.

  • We do not currently use electronic medical records (however, a copy of your written records are available upon your request).

  • We will not list your information in a hospital directory.

  • We will never use your personal information in marketing materials.

  • We will ask for your written permission to discuss identifiable information with other health professionals (except in the case of an emergency where we are concerned about your safety or the safety of others).

  • We will not use any identifying information (names, addresses, phone numbers, or any other information that might make your identity easily known) for research purposes.

  • This practice is not involved in the procurement or donation of organs or tissue.
